Patient Toolbox
The Patient Toolbox section of the website allows you, the
patient, to carry out a variety of activities from the comfort
of your home, 24 hours a day, 7 days a week, 365 days a year.

Request Prescription
This option allows you to request your repeat prescription,
online, whenever you want to. This service has been very
successful Just fill in your details and click send. Please
note that this service should not be used to request
medication that is not found on your repeat medication slip
that comes with your prescription.
Health
Questionnaire
Many of you will receive a Health Questionnaire through the
post from time to time. The aim of these questionnaires is to
gain health information on yourself, without asking you to
come to the surgery taking up your time.
We
have now added this to the website to further simplify this
process. Now all you need to do is fill in the details and
click send, no need to even go to the post box!
Update Your
Details
Having up to date information on our patients is vital. If you
have changed your address, name or phone number, or any other
information please do let us know.

Health Centre cannot accept liability for any failed requests
or submissions.
If you decide to request prescriptions, or use any other
service available on this website, you will have to enter
information about yourself that will be sent to us across the
internet. This information will not be encrypted and is as
safe as normal e-mail. We cannot guarantee that this
information will not be seen by others.
