How to Register
Please
complete and sign one of the NHS Registration Forms which are
available at Reception. We will ask you to fill out a Health
Centre form with practical questions which are of help to the
practice while we are looking after you.
We strongly
recommend our new patient check for all patients over five
years and will therefore ask you to make an initial
appointment with the Practice Nurse. This allows the practice
to obtain base line data on your health needs. If you are
taking regular medication, please make an appointment to see
the doctor before the next prescription is due.
Having registered
with the practice you are free to see any doctor. however we
do encourage patients to stay with one doctor for any
particular medical problem. This is particularly important in
chronic disease, for continuity of care.
Patients with Particular Needs
We have a
parking space directly in front of the Health Centre which is
reserved for patients displaying a disabled badge. Wheelchair
access onto the path is midway along the long side of the
Health Centre. There is no lift to the first floor of the
Health Centre but we try to ensure that disabled patients are
seen on the ground floor and a ramp makes the Health Centre
annex accessible. There are toilets available for people using
wheelchairs and a baby changing facility within the main
Health Centre toilets. Our Practice Leaflet can be made
available on request in Braille, on audio cassette tape and in
large print.

We offer the
direct access appointment system developed by the Government.
We attempt to offer most appointments on the same day of the
request but patients can expect to have an appointment by the
end of the following working day. Urgent cases are always seen
on the same day. We offer a limited number of advanced
bookings in recognition that some people have to organise in
advance. The busiest days are Mondays, Fridays and the day
after Bank Holidays. If your appointment is not really
necessary on these days it would help if you could book on
another day. Please do not arrive in the middle of surgery
without an appointment, unless it is an emergency - this only
adds to delays for the patients who have booked their
appointments. Please telephone first.
