We are fully
registered with the Data Protection Act and give the utmost
priority to confidentiality of medical records, both on
computer and paper.
Practice Area
Our Practice area includes Ivybridge, Cornwood, Lutton,
Ermington, Ugborough, Bittaford and Lee Mill. For a detailed
map of the area, please see our reception staff.
Change of Address
Please notify us immediately if there is any change to your
name, address or telephone number. This is important in order
that the information held is up to date and that all
correspondence is sent to the correct address. Also remember
to inform any Hospital where you may be receiving ongoing
care.
Home Visits
If you are too ill to come to the Health Centre a home visit
can be requested by Telephoning (01752) 690777. It is helpful
if you can telephone before 10.30am so that the Doctors can
plan their rounds before leaving the Health Centre. Explaining
what is wrong to the receptionist will also help Doctors
Prioritise their visits.
Speaking to your
Doctor on the Telephone
We have recently introduced telephone consultations. If
appropriate, this enables you to speak to your doctor by
telephone rather than making an appointment to be seen in
surgery. The receptionist will take your telephone number and
a few relevant details, the doctor will then ring you back
during that day. Results of tests/investigations are available
through the receptionists during the afternoon.
Repeat Prescriptions
If you need repeat medication your Doctor will have arrange
for your prescription to have a tear off slip with your repeat
medications listed. Before your drugs have run out please tick
the required items on this slip and leave it in the
prescription box at reception. Please give at least 2 working
days notice for your prescription to be prepared for
collection. Arrangements can be made for your prescription to
be forwarded to the local pharmacy of your choice, enabling
you to collect your medication directly from them. To avoid
dangerous errors we regret we cannot take requests for repeat
prescriptions over the telephone.
When the Health
Centre is Closed
If you need a Doctor in an emergency telephone (01752) 690777
and a recorded message will give a choice of the number for
NHS Direct or the number to contact a Doctor covering Practice
emergencies. Telephone calls to this service are recorded. A
Doctor will be available to give advice, or you may be asked
to attend a treatment centre in Plymouth or Totnes. If however
you are unfit to travel, a home visit will be arranged.
In an emergency
where it would be dangerous to wait for a Doctor an ambulance
can be called by dialling 999.
NHS Direct is a
nurse-led 24 hour helpline that is available by calling 0845
4647. NHS Direct is a useful source of advice for patients.
Branch Surgery
There is a very limited service, dealing with minor ailments
and prescriptions, held at Cornwood Community Room on the 1st
Thursday each month. Please contact the Health Centre on
690777 prior to attending this surgery.
Car Parking
We would be grateful if patients would only park in the Health
Centre car park while actually using the Health Centre
services. Car parking is at a premium but there is usually
adequate car parking in the surrounding roads and in the
public car park in Harford Road. The ill, frail, elderly and
disabled patients have priority, therefore if you are well,
please consider parking elsewhere.
